Forest Preschool – Sprouting Knowledge

2023/24 Sprouts Registration

Our preschool age program costs $360/day. This is $30/class for each day registered. Two days a week is $720 per session. We happily accept whatever payment terms work for you.

This is a long form and we know you are busy. It is really important that you read it thoroughly and complete it accurately so we have given you the opportunity to save it and return should you get partway through.

At the end of completing this form you will be asked to e-transfer krystal@sproutingknowledge.com a $50 non-refundable deposit to secure your spot. Thanks!

By registering your child for one of our programs you are agreeing to the Terms & Conditions of service set out below:

I understand that I am responsible for reading the Parent Handbook and making myself familiar with the policies of Sprouting Knowledge located here: https://sproutingknowledge.com/policies

Medical Consent
I give my consent that my child may be given first aid/emergency treatment by the qualified staff at Sprouting Knowledge operated by Krystal Jeffrey.

Photography Release
I understand photos and videos will be taken of the children for use in Sprouting Knowledge marketing and advertising materials. Sprouting Knowledge will never explicitly identify children by name or age. Usage may include but is not limited to: Facebook, Instagram, our website, newspaper articles, and brochures.

Drop Off & Pick Up Acknowledgement
I understand the staff at Sprouting Knowledge assumes responsibility for the child once the caregiver make eye contact with a teacher and the child has been greeted. The responsibility of Sprouting Knowledge ends when the child has seen the adult responsible to pick up the child. All children will be signed in upon arrival and will be signed out at the end of the day.

Acknowledgement & Assumption of Risk(Required)

Although we do everything we can to prevent injury, learning outdoors carries with it a certain level of risk. This is an outdoor program and nature is seldom predictable. At Sprouting Knowledge we encourage the children to take supported risks. In this way, they learn how to manage risk in a responsible manner.

I acknowledge and agree that I am aware that there are many known and unknown dangers and risks associated with participation in the outdoor activity program of Sprouting Knowledge including, but not limited to, the risk of minor injuries such as cuts, bruises, and scrapes, insect bites, insect stings. I am also aware that there is also the risk of serious injury or death.

I acknowledge and agree that I understand the dangers and risks associated with the outdoor activity program of Sprouting Knowledge and, notwithstanding such dangers and risks, I give my consent for the Child to participate in the outdoor activity program of Sprouting Knowledge.
I release, waive, and fully discharge Krystal Jeffrey doing business as "Sprouting Knowledge," her Employees, Contractors, Agents and successors in business (the "Releasees") from any and all claims, demands, obligations and liabilities of any kind or nature whatsoever arising from or connected, directly or indirectly, with the Child's participation in the outdoor activity program of Loving Learning due to any cause whatsoever, including negligence, breach of contract, and breach of any statutory duty of other duty, and I accept that this release covers and includes, but is not limited to, all unknown and unforeseen claims, injuries, damages and losses, and any consequences thereof.
I will indemnify and hold harmless the Releasees from any damage, loss, liability, legal costs and other expenses that any of them may suffer or incur by reason of any claims against any of them arising from or connected, directly or indirectly, with the Child's participation in the outdoor activity program of Sprouting Knowledge, including any claim based on negligence, breach of contract, and breach of statutory duty or other duty.

IN CONSIDERATION OF MY CHILD BEING ALLOWED TO PARTICIPATE IN THE SPROUTING KNOWLEDGE PROGRAM, I agree to the above assumption of risk and waiver of claims.
